Columbia Cares for You is a group of health professionals in Columbia County dedicated to raising awareness of dementia and addressing the specific issues surrounding the needs of dementia patients and their families in our community. CC4U hopes to provide the latest information and resources to healthcare professionals as they treat patients with dementia and to support caregivers in the home through education and training initiatives.
The primary goals of CC4U are to plan and coordinate long-term and support services, so that rural elders are able to overcome traditional barriers such as lack of awareness of resources, incomplete services, and long distances to travel for services.
The ultimate goal of the group is to link services more effectively for rural elders in Columbia County and surrounding counties thus improving quality of life and helping them stay as independent as possible for as long as possible.
CC4U is made up of healthcare and social service professionals from Columbia County Senior Services Inc, the Department of Veterans Affairs, and the National Association of Retired Federal Employees.

ColumbiaCares4U.org is a Blue Foundation grant project that provides patients, caregivers, and healthcare professionals with the knowledge, training, and support for dealing with Alzheimer's and related dementias.
